Rhinoplasty: Frequently asked questions

Nasal Plastic surgery is a specialty in itself. It has its own niche and unique questions. I will highlight the 9 most common clients concerns.

Who is the ideal candidate for a nose job?

Ideally anyone over the age of 18 years, in good medical health who requires correction of a cosmetic nasal defect. Improving the nasal profile and projection or realignment of broken bones is a common request. This is called a reduction rhinoplasty. Some nasal profiles show a significant "saddle" deformity either from previous trauma, ethnic traits or inherited facial characteristics. This is known as "augmentation" rhinoplasty. It will require a cartilage graft either from the patients nasal septum or their auricular (ear) cartilage to cover the saddle shaped defect.

Rhinoplasty was usually for Europeans or North Americans. Nowadays it is not unusual for clients of different ethnicity to request nasal plastic surgery. The cosmetic requirements need surgical adaptation. The Ethnic variation of rhinoplasty is the adaptation of surgical and aesthetic principles to different racial characteristics.

Those clients unfortunate enough to require a second procedure present the rhinoplastic surgeon with a unique and difficult problem known as revision or secondary rhinoplasty. The scarring from previous surgery and the expectation from the dissatisfied client together with technical difficulties make this a very demanding and challenging procedure.

Nasal tip surgery is often needed for refinement. It takes many years to master rhinoplasty, many plastic surgeons simply do not speciaize highly enough to achieve this level of expertise.

2) How is it done?

Surgery may be performed using "Twilight" anaesthesia or general anaesthesia depending on the client's choice. The majority of cases are performed utilizing internal incisions that are not visible and hence no external scars. This is called a closed rhinoplasty. Sometimes it is required to make an external incision in the columella ( the partition between nostrils). This is called the open or external approach. It depends what needs to be done to the nose; for example, when placing a cartilage graft or refined tip work.

3) How long will it take?

This depends on the type of rhinoplasty, but as a rule between one to two hours.

Whatever the results you expect they have to be realistic and achievable. The consultation is paramount to success?

It is vital to have realistic expectations and this is where the preoperative consultation is so important. Both client and surgeon have to sing from the same hymn sheet so to speak. Remember your surgeon wants to make you happy, his livelihood depends on it.

5) What is the recovery?

Rhinoplasty is not usually painful, but that doesn't mean it is not uncomfortable. With common sense and no complications most patients can return to work within a fortnight. Avoidance of competitive extreme sports where there is a high risk of facial injury. If you are not a professional sportsperson, many surgeons advise not to partake in contact sports that are over competitive!

6) What risks and complications are involved?

Bleeding is a common complication but usually stops rapidly. Approximately 10% of patients may require a minor adjustment at a later stage. 7) Are there any after effects following Cosmetic Rhinoplasty?

Bruising and black eyes are so common as well as nasal blockage, light seepage of blood, temporary loss of smell and swelling. These are transient and should return to normal after several weeks.

8) Can the rhinoplasty be combined with other procedures?

It can be done in conjunction with a septoplasty i.e. straightening the nasal cartilage partition. It is also possible to have a chin augmentation or lip enhancement performed simultaneously.
